Job Description:
* Case Manager coverage needed for 3-4 weeks
* Must have experience working with patients in a hospital environment
* Work with patients/families identified to have discharge planning, psychosocial, financial or other needs to develop a plan that will:
1. enhance functional and self-care
2. assist utilization of services
3. facilitate communication and coordination of efforts among health care team members
4. promote quality, cost effective outcomes

Requirements:
* 3 years Case Management experience preferred to include UR, resource management or discharge planning
* Registered Nurse with current Texas License
* Available to start immediately
